Items not being taken in quests?

I've had this happen a few times, I usually don't notice until I go to quick stock and then price items so I'm not 100% sure how often this has happened. I know it's happened once on an oracle quest and possibly a couple of times on the wizard quests and saggitarius. I remember buying the item, I know I completed the quest and I'm sure I didn't receive the same items as prizes, but they're still in my inventory. Has anyone else had this happen?

ETA: Whoops! Forgot, I'm running Windows Vista and Firefox.
