Before his fateful encounter with Thassarian and his resurrection as a Death Knight, Koltira lived in the hidden High Elven outpost of An'owyn with his brother Faltora and Merriel, another High Elf. Along with An'telas and An'daroth, An'owyn was one of the three High Elven locations that guarded a piece of the moon crystal that made up the Key of the Three Moons. The key, when fully formed, was used to bring down Ban'dinoriel, the second Elfgate of Silvermoon City.

When the Scourge assaulted An'owyn, a dagger struck Merriel in the eye and she was killed immediately. Filled with rage and grief, Faltora charged the leader of the assault directly while Koltira watched on, pinned down by the Scourge and helpless to offer assistance. The leader of the assault -- the Death Knight Thassarian -- defeated Faltora with ease and was about to finish him, but he hesitated and allowed Faltora to live. This action simultaneously confused Koltira and piqued his curiosity. Faltora was brutally murdered by an Abomination shortly after Thassarian spared his life, but Koltira managed to escape the assault. He followed Thassarian and confronted him several hours after the attack. Koltira could feel the conflict in Thassarian and gave him toe offer of betraying the Scourge and joining his forces to fight against them. Thassarian refused, but chose to spare Koltira's life until the next time that they met. The next day, the Elfgates of Silvermoon were breached by the Scourge and Koltira and Thassarian continued their duel. Thassarian emerged victorious and murdered Koltira. Koltira was then raised as a Death Knight. Although Thassarian was Koltira's murderer, the two share a close bond and refer to one another as brothers.

During his time at the floating Scourge necropolis named Acherus, Koltira was captured by the fanatical forces of the Scarlet Crusade and kept prisoner in the Scarlet Hold. He was freed when Thassarian sends Death Knights to his aid. Saved from the grasp of the Scarlet forces, he is sent to the offensive on Light's Hope Chapel. The plan was to raise the soldiers buried under the chapel into Scourge servitude, but Tirion Fordring, leader of the Argent Crusade, cleansed the Corrupted Ashbringer sword and used it and the Holy Ground to his advantage to drive The Lich King back. The Death Knights present at the battle were freed from the control of the Lich King's grasp and Darion Morgraine, the Leader of the Death Knights present at the battle went on to form the Knights of the Ebon Blade.

After Koltira parted ways with Thassarian, he joined the New Horde after convincing Warchief Thrall to accept the former Blood Elf, Troll, Tauren, Forsaken and Orc Death Knights as allies. Despite the fact that Koltira was born a High Elf, many of his brethren were now Blood Elves and that apparently led him to join the Horde.

After the Cataclysm, Koltira is sent to reclaim the city of Andorhal for the Forsaken. This attempt to reclaim the city turns into a three-way battle for control between the Alliance (led by Thassarian), the Forsaken (led by Koltira Deathweaver) and the Scourge (led by Darkmaster Gandling and Araj the Summoner). After the Scourge leaders are defeated and their minions dealt with, Thassarian and Koltira enter a truce. The truce is broken by a sneak attack on Forsaken forces by Alliance farmers, and the war between the Alliance and the Forsaken is reignited. Koltira begins the counterattack, and requests the heroes of the battle to seek an Undead named Lindsay Ravensun, of whom he seems to hold a very low opinion. This person is revealed to be the leader of the Forsaken, Sylvanas Windrunner, in disguise. She sends Horde agents assisting in the battle to battle the Alliance with Val'kyr -- former agents of the Lich King with the power to resurrect the dead into undeath -- and forces the Alliance to retreat. After the final battle, Koltira is confronted by Sylvanas, who scolds him for being weak and for agreeing to the truce with Thassarian. She opens a portal to Undercity and Koltira is pulled through it by a hooked chain. Sylvanas reveals that she intends to erase Koltira's compassion for Thassarian, and make him a better servant to the Horde... specifically, to herself and the Forsaken. Koltira has not been seen since.

In-game, Koltira can be found in Acherus: The Ebon Hold in Eastern Plaguelands, in Agmar's Hammer in Dragonblight, aboard the Horde airship Orgrim's Hammer in Icecrown, and in Andorhal in Western Plaguelands.
