Otto Octavius was born in Schenectady, New York, USA. His childhood was turbulent, as he was abused by his father, Torbert, and bullied at school. Otto's mother Mary, however, encouraged her son's academic talents, and he excelled in all phases of his education, culminating in a PhD in nuclear physics from the Massachusetts Institute of Technology. Torbert Octavius died in a construction accident in his son's first year at MIT.

After graduating from MIT, Octavius became a researcher at the US Atomic Research Center. In the course of his work at the UARC, Octavius invented a multi-armed chest-mounted harness, meant to allow the wearer to handle radioactive material more safely; this earned him the nickname of "Doctor Octopus" among his colleagues. During this time, Octavius' mother died of a heart attack while arguing with her son, allegedly over her behaviour regarding both Octavius' respective romantic lives.

The loss of his mother and his subsequent guilt are cited as a contributing factor in an accident Octavius suffered shortly after: while using his harness in the UARC laboratory, Octavius dropped the radioactive materials he was working with, causing a massive atomic reaction that fused the harness to his body and altered his brain chemistry. The damage to his brain allowed Octavius to control his mechanical arms with his own brainwaves, but also seems to have radically changed his personality -- his first act as the supervillain "Doctor Octopus" took place immediately after awakening in a hospital after the accident, where he took the hospital staff hostage and attempted to force them to do his bidding. After this scheme spread to a local nuclear facility, it was halted by Spider-Man (AKA Peter Parker, SHIELD file 628.7).

Upon his release from prison after this incident, Octavius appears to have become obsessed with Spider-Man, at one point committing crimes across the United States in an effort to lure Spider-Man from New York City. While incarcerated again, Octavius' harness was surgically removed from his body; authorities believed this removed the majority of the threat he posed, but Octavius demonstrated long-distance telepathic control of the arms and used them to escape from the prison.

After this escape, Octavius formed the first incarnation of the supervillain team known as the Sinister Six (SHIELD file 906.6), which consisted of himself, Electro (AKA Maxwell Dillon, SHIELD file 654.5), Kraven the Hunter (AKA Sergei Kravinoff, SHIELD file 678.4), Mysterio (AKA Quentin Beck, SHIELD file 663.2), Sandman (AKA William Baker, 662.7), and the Vulture (AKA Adrian Toomes, SHIELD file 649.1). Octavius functioned as the group's leader. The group disbanded after being defeated by Spider-Man, but various incarnations of it have continued to appear since.

Octavius formed another group, which he led under the alias of "the Master Planner" in an attempt to develop an atomic weapon with which to control the Earth; this plot was once again stopped by Spider-Man. He also attempted to marry Spider-Man's aunt, May Parker (SHIELD file 1204.5), in order to gain control of a nuclear reactor, but was stopped by her nephew before either of these goals came to fruition.

Octavius was murdered at one point by the clone of Peter Parker known as Kaine (SHIELD file 628.9), but was resurrected by the organization known as the Hand (SHIELD file 575.8). After his return to life, Octavius was brainwashed into attempting to kill the Green Goblin (AKA Norman Osborn, SHIELD file 636.4), but was unsuccessful. Octavius worked with the Thunderbolts (SHIELD file 1009.3) for a short time after the period known as the Civil War (SHIELD file 2005.1), but quit and was apprehended by SHIELD (SHIELD file 1.1).

After escaping from SHIELD containment, Octavius realized that his body was suffering from multiple terminal conditions after years of battling Spider-Man, which eventually led to Octavius trading bodies with Parker shortly before the death of his original body. Octavius proceeded to live as both Parker and Spider-Man and eventually Venom (SHIELD file 913.6) after removing the symbiote from Eugene "Flash" Thompson (SHIELD file 625.1) during a medical procedure. Octavius eventually relinquished control of Parker's body to a remnant of the consciousness of its original inhabitant in order to protect Anna Maria Marconi (SHIELD file 1583.4), a woman Octavius had fallen in love with during his time as Parker.
